You have reached the Continuing Education (CE) information page for programs sponsored by the Florida Center for Public Health Preparedness (FCPHP), University of South Florida and Suwannee River Area Health Education Center (SRAHEC). |
Thank you for participating in the program trainings for Field Epidemiology and/or Disaster Behavioral Health. (Please note: Disaster Behavioral Health programs will be available in Spring 2008). Each participant seeking credit must complete the following items prior to receiving the CE Certificate:
Must complete appropriate on-line pre-requisite courses (If applicable)
Must attend on-site programs
Must complete the SRAHEC Registration Form with applicable license number
Must complete the on-site evaluation for SRAHEC and the on-line evaluation for the FCPHP
Must obtain a passing score (75% or greater) for on-line program post-test. Instructions provided by FCPHP
CE CERTIFICATES
Certificates will be generated based on the successful completion of the items listed above. Please allow 2 weeks for delivery of your certificate from the time that you complete your post-test. All participants seeking CE credit must complete the on-site SRAHEC CE Registration form as this will be considered your request for CE credit.
